HILUX YIP 343 $0.45 per km $0.55 per km These vehicles are now registered in the names of either the Heads of Department or other individuals who have ultimate responsibility for them. By borrowing a vehicle, YOU are accepting responsibility for any speeding fines or traffic infringements incurred during the period this University vehicle was booked out in you name. You must also return the vehicle clean and fully refuelled (not applicable to short metropolitan trips). Be sure to fill out logbooks accurately and when using fuel card, you must state the vehicle mileage to the console operator. The mileage charge covers only fuel and normal tyre wear (all damage to tyres will be charged to the user). Damage to vehicles must be reported, however minor. Insurance excess or cost of repairs/replacement may have to be met by you. You must provide us with a photocopy of your driver's license. The fleet managers reserve the right to allocate vehicles to particular fieldtrips as required and to alter bookings if necessary, users do not have the right to insist on a particular vehicle. Any use of these vehicles without the booking/acceptance form being completed will be regarded as vehicle theft and will be reported to the Police.

This equipment belongs to either specific Departments or Individuals and should be treated with the utmost care and respect. Some of this equipment is hazardous to use either in a physical or chemical sense and so please read the guideline for use, associated with each piece of equipment you use. Proper training will be provided or a experienced operator will accompany the user.

By submitting this form, YOU are accepting responsibility for this equipment. Be sure to fill out logbooks accurately, reporting any disposables used, irregular operation or any damage that may need to be repaired after your use. Insurance excess ($500.00) in case of lose through theft will be met by you. You must provide us with a photocopy of your driver's license. The equipment managers reserve the right to deny usage and to alter bookings if necessary. Removal of this equipment without the booking/acceptance form being completed and the managers being informed will be regarded as theft and will be reported to the Police.
